Required Documents

(1) In case of re-issue of passport due to its expiration or finishing of its pages:-

Printed copy of online filled application form.

ICAO compliant photograph should be uploaded and two copies of it be submitted along with the application. One of the photographs should be fixed on first page while the other need to be pasted at the blank space of the third page of the application form.

Self-attested /signed photocopies of all the pages of the current passport, having information about the applicant including endorsements in any, should be submitted with the application. Self-attested copy of Permanent Residence card is also required. Where applicant is not a PR, current immigration document (e.g. Student ID, Work Permit, etc.) are to be submitted.

If the applicant is a student, details of educational institution, along with period of study is required. In case of work permit holders details of employer and period of employment is required.

(2) In case of re-issue of passport due to its loss:-

In addition to the above-mentioned documents, original police report mentioning date and place of loss of passport along with the passport number.

(3) In case of re-issue of passport due to damage:-

In addition to documents mentioned in point (1) above, an affidavit form (Annexure ‘F’) the applicant regarding loss of passport is required. The proforma may be seen here.

(4) In case of re-issue of passport due to change of name, the following documents are required, in addition to those mentioned in point (1) above:

Gazette notification or original newspaper advertisement announcing change of name, published in two leading daily newspapers. The newspapers should be of area of applicant’s permanent address in India (one in English and other in Hindi/local languages). Waiting time of 30 days is mandatory after the publication. 

Two public documents issued either by Indian Government including State Government/school certificates or local Government/local school documents in respect of applicants living abroad. 

Affidavit form (Annexure E) regarding change of name, in prescribed format signed in the presence of Consular Officer. The proforma may be seen here

(5) In case of re-issue of passport due to change in personal particulars other than name, the following documents (original and copy are required in addition to those mentioned at point (1) above:

In case of change of address – Proper address proof such as Aadhar Card, Voter ID, House Registration, Tax Receipt, Electricity/Telephone bill or Nativity Certificate issued by the Competent Authority of the State concerned.

In case of addition of spouse name – Apostilled marriage certificate. 

In case of deletion of spouse name – Divorce decree from the court

In case of change of surname due to marriage – Apostilled marriage certificate. 

(6) In case of minor passport, an affidavit form (Annexure ‘D’) has to be filled and signed by both parents at the time of application. The proforma may be seen here.

(7) In case of new born baby, printed copy of online filled Indian Citizenship Form, printed copy of online filled Passport Application Form, printout of acknowledgment receipt generated on SEWA portal, parents’ passports, parents’ visas/residence permits, parents’ marriage certificate, child’s birth certificate, two photographs (5”x5”, white background), affidavit (Annexure D) signed by both parents for minor’s passport. All the documents are required in original for verification along with two sets of photocopies.

Note:- Applicant is required to upload all required documents after submitting the online application as well as bring original and one self attested photocopy of the document while visiting the Embassy for submission of application.
