Required Documents and Procedure

Indian passport and Czech visa/residence permit (originals and copies) and mandatory for each and every application.

For any consular service other than the Police Clearance Certificate, applicant has to apply through SEWA portal and follow the procedure as mentioned at the following link: https://indianconsularservices.mea.gov.in/consularServices/ 

After filling the Form, the applicant is required to visit the Embassy for submitting the applications and admissible consular fees on any working day between 0930 and 1200 hrs. List of Holidays | Embassy of India, Prague, Czech Republic  may also be seen before coming to the Embassy.

For General Affidavit/Power of Attorney, the applicant has to bring the requisite  Affidavit/Power of Attorney typed on a plain A4 paper.  Applicant will affix his/her signature with current date and place on such Affidavit/PoA at the Embassy counter in the presence of the Consular Officer.

For attestation of photocopies of documents, originals must be presented at the time of application.
