Required Documents and Procedure

Indian passport and Czech visa/residence permit (originals and copies) and are mandatory with each and every application.
To obtain any consular service other than the Police Clearance Certificate, a Miscellaneous Services Form needs to be filled, signed and submitted to the Embassy along with necessary documents and one photograph. The Form may be found here.

After filling the Form, the applicant may come to the Embassy for submitting the applications and admissible consular fees on any working day between 0930 and 1200 hrs. List of holidays may also be seen before coming to the Embassy.
In case of General Affidavit/Power of Attorney, the applicant has to provide, along with the application, the required Affidavit/Power of Attorney typed on a plain A4 paper and singed by the applicant. Applicant will affix his/her signature with current date and place on such Affidavit/PoA at the Embassy counter in the presence of the Consular Officer.
For attestation of photocopies of documents, originals must be presented at the time of application.
